General comments

Lokaal beleid Flevoziekenhuis

1.     Bepaal de BMI en de klaring

2.     Algemene regel: geen aminoglycosiden bij klaring < 30

3.     Duur gebruik aminoglycosiden

        maximaal 2 dagen

        Uitzonderingen alleen in overleg microbioloog/infectioloog

Spiegelcontrole Flevoziekenhuis

 Topspiegel 1 uur na start infuus

 Dalspiegel 23 uur na eerste gift (1 uur voor 2e gift), dit is een cito-spiegel

 NB. Registreer tijd van bloedafname en tijdstip toediening aminoglycoside! De monsters dienen door de arts of verpleegkundige te worden afgenomen i.v.m. de wisselende tijden van monstername. De bepalingen worden cito gedaan bij de Klinische Chemie.

 Interpretatie
 Dalspiegel ≤ 1mg/L:   2e gift  zelfde dosering 24 uur na de eerste gift
 Dalspiegel > 1mg/L:   2e gift minimaal 12 uur uitstellen en overleg ziekenhuisapotheker

BMI < 25
6 mg/kg, mits klaring > 30 

BMI 25-40
6 mg/kg, mits klaring > 120*
5 mg/kg, mits klaring 90-120
geen gentamicine bij klaring 30-90 

*Ga, indien CKD-EPI >90, in SAP op het veld staan met de cursor, dubbelklikken, dan wordt de gemeten waarde zichtbaar

BMI > 40
Alleen gentamicine bij klaring >= 90 (landelijk grens bij 60)
Gewicht < 100 kg: 5 mg/kg 
Gewicht >= 100 kg: aangepaste dosering per gewichtscategorie (range 3,6-5 mg/kg TBW), zie landelijk beleid.

100 - 120 kg: 480 mg 1x per dag
120 - 140 kg: 560 mg 1x per dag
140 - 160 kg: 600 mg 1x per dag
160 - 180 kg: 680 mg 1x per dag
180 - 200 kg: 760 mg 1x per dag
200 - 220 kg: 800 mg 1x per dag
